Welcome to New Town Dentistry! Dr. Douglas Wolfe and his team have a vision of providing outstanding dentistry and service in a comfortable environment for you! We strive to provide this experience for each and every patient. When you schedule an appointment with us we will reserve Dr. Wolfe, the team and the treatment room for you. In order to provide this customized level of dentistry and service, here is some helpful information.
If you must change your scheduled appointment time, please contact us as soon as possible or at least 2 business days prior so we can accommodate other patients. Please respect our access to care for all our patients.
We ask our new patients to print out and complete our “important forms” under the “patient information” tab before you arrive. If you cannot we ask that you arrive 15 minutes prior to your reserved time to complete this paperwork. If you are more than 10 minutes late we may reschedule you in order to serve all our patients on time.
If you have recent radiographs (x-rays) or dental records, please bring them or have them forwarded to our office by email or traditional mail. Due to privacy law you must sign a “release of information” for your previous dentist. We have this form listed under “Patient Information” / “Important Forms” / “Release of Information.” Please complete and sign and give this to your previous dentist so your information can be released to New Town Dentistry.
If you are pregnant or have a serious medical condition that may affect your dental care please let us know. We may ask you to have your physician complete a medical consult form before we initiate treatment.
We ask that during treatment all family or friends wait in our reception area. Also, if someone other than the parent or guardian is bringing a minor for an appointment, we must have a signed consent on file. This is also found under our important documents labeled “Authorization for Minor.” We also ask that the parent or guardian does not leave the office during treatment of a minor.
Furthermore, if you have a mobile phone, please set it to silent and refrain from using it in the office. This enables us to provide exceptional individualized care for every patient.
If you have any questions or concerns please let Dr. Wolfe or any of the team members know. We will do our best to help you.
